This is for the information of all concerned that those who have already registered themselves under the West Bengal Health Scheme (WBHS) Portal are required to update their general information and family details, as the WBHS Portal has recently changed to new digital system.

Due to prevailing uncertainties emerging out of pandemic crisis, all concerned are advised to initiate the updating procedure as early as possible. Those who have not initiated the updating procedure till now, are requested to initiate the process by e-mailing the following information with substantive documents to wbhs@snggdcg.ac.in

List of required information/documents:

1.                  Cover letter addressed to the Officer-in-Charge (clearly mentioning whether there is mere updating of existing beneficiaries or any other addition/deletion of beneficiary/ies as well)

2.                  WBHS application ID (e.g. WB/EMP/XX/XXXXXXXXX)

3.                  GPF no. of the employee

4.                  Name of each beneficiary

5.                  Date of Birth of each beneficiary

6.                  Latest pay slip

7.                  Pay level according to Ropa 19

8.         Recent digital or scanned photograph and signature, blood group of each member including the incumbent.

9.                  HRMS ID

10.              IFSC code of the Salary Bank A/c Branch

11.              Salary Bank A/c no.

12.              Birth Certificate of the newborn (if applicable)

13.              Monthly income (if any) of each beneficiary 

14.              For addition/deletion of beneficiary/ies, please follow Serial Nos. 1, 4, 5, 8, 12 (for newborn) and 13.
