The undersigned wishes to seek your kind attention to the serial Govt Orders 105/Secy (H & FW)/2021 dated 18.5.2021, 106/Secy (H & FW)/2021 dated 18.5.2021 and 108/Secy (H & FW)/2021 dated 19.5.2021 recently issued by the Secretary, Department of Health and Family Welfare, Govt. of West Bengal and circulated by the Higher Education Department (H.E.D.) in connection with urgent appraisal of vaccination status amongst the college staff members.

In line with the prescribed directives of H.E.D., two database formats will be circulated by the college office, one for the Age Group 18-44 years and the other for the Priority Age Group 45 years & above.

The staff members are advised to fill-up the particular Form relevant to their respective age group and send back the duly signed copy to college office at the mail-id office@snggdcg.ac.in positively within 24.5.2021 (Monday) 12 Noon.
